ABSTRACT

ObjectiveTo clarify the development and methodological research status of clinical comprehensive evaluation of Chinese patent medicines in China and identify the problems and difficulties in the evaluation, so as to provide a reference for promoting the subsequent evaluation. MethodFirstly, we analyzed the current situation of clinical comprehensive evaluation in China by reviewing the articles about the process of clinical comprehensive evaluation of drugs and the process of clinical comprehensive evaluation of Chinese patent medicines. Secondly, we comprehensively summarized the formulation background and key points of policies related to clinical comprehensive evaluation of Chinese patent medicines and then show the development status in this field at the national and provincial levels. ResultThe comprehensive clinical evaluation of Chinese patent medicines is still in its infancy in China, and 32 articles of specific evaluation of Chinese patent medicines were included in the study. The dosage forms were mainly capsules (15 articles, 46.88%) and injections (28.13%). The evaluation mainly involved diseases such as the nervous system (10 articles, 31.25%), digestive system (5 articles, 15.63%), and respiratory system (5 articles, 15.63%). The research results of clinical comprehensive evaluation of Chinese patent medicines were mainly from relevant societies and research institutions. Different research teams have developed technical guidelines or specifications for the evaluation, while the government-leading evaluation guidelines remained to be formulated at the national and provincial levels. In addition, the research articles in this field mainly concentrated on the application of evaluation methods, the building of evaluation index systems, and completed evaluation reports. ConclusionTo reflect the unique value and advantages of Chinese patent medicines, the government needs to build technical guidelines for the clinical comprehensive evaluation of Chinese patent medicines on the basis of the clinical comprehensive evaluation of drugs and create a favorable policy environment for the evaluation work.

ABSTRACT

BackgroundThe etiology and pathogenesis of somatic symptom disorder are complicated, for which no effective treatment currently exists, posing a critical impact on their quality of life. ObjectiveTo analyze the current situation and development trend in domestic and foreign research on somatic symptom disorder from 2011 to 2021, and to better understand the research frontiers and hot spots in this field. MethodsOn 31 August 2022, literature on somatic symptom disorder published from January 1, 2011 to January 1, 2023 were searched in China National knowledge Infrastructure (CNKI), Wanfang Data Knowledge Service Platform, Web of Science and PubMed, and a total of 15 035 articles were included, then the visualization analysis of the number of publications and keywords was conducted based on Microsoft Excel and CiteSpace software. ResultsIn the field of somatic symptoms disorder research from 2011 to 2021, the number of annual publications demonstrated an overall upward trend in foreign studies, and displayed a slow downward trend in domestic studies. The keyword co-occurrence network analysis revealed that the top five high-frequency keywords were "mental health" "depressive disorder" "depression" "anxiety" and "somatic symptom" in domestic studies, and were "symptom" "prevalence" "depression" "disorder" and "quality of life" in foreign studies. The burst analysis suggested that the burst terms in past five years were "somatic symptom" "syndrome" and "quality of life" in domestic studies, and were "major depressive disorder" and "outcome" in foreign studies. The clustering analysis yielded 8 clusters in domestic studies and 4 clusters in foreign studies, reflecting in the related disease, prevention and treatment protocols and epidemiological characteristics of somatic symptom disorder. ConclusionIn recent years, the publications and academic concerns regarding the research on somatic symptom disorder show an upward trend in foreign studies and are relatively inadequate in domestic studies, and both the domestic and foreign studies have been focused on the influencing factors of somatic symptom disorder. ABSTRACT

Objective:To analyze the related literature of pulmonary rehabilitation research at home and abroad, understand its research focus and development trend, and provide a reference and basis for future pulmonary rehabilitation research.Methods:Literature related to pulmonary rehabilitation published in the database of Web of Science and China National Knowledge Infrastructure from January 2015 to February 2022 was retrieved, and the contents such as the number of articles published annually, authors, research institutions, and high-frequency keywords were visually analyzed by bibliometrics and CiteSpace software.Results:The number of articles published each year showed an obvious upward trend from 2015, and the number of Chinese documents was higher than that of English documents from 2019 to 2020. MARTIJN A SPRUIT was the author with the largest number of articles published in English, with a total of 65 articles published, with close cooperation among scholars. Che Guowei was the author with the largest number of articles published in Chinese literature, with 13 articles published in total. The cooperation among the authors was relatively lacking and scattered. The main research institutions abroad were universities, and the institution with the highest number of papers was Maastricht University in the Netherlands, with a total of 85 articles. The research institutions cooperated closely and formed a cooperation network. Hospitals were the main research institutions in China, and the thoracic surgery Department of West China Hospital of Sichuan University had the highest total number of publications, with 14 articles. The cooperation among institutions was not close, and there was a lack of cooperation with universities and scientific research institutions. Through keyword co-occurrence and clustering and timeline view analysis, it was concluded that the research hotspots in this field were pulmonary rehabilitation of patients with different chronic respiratory diseases, the influence of pulmonary rehabilitation on lung function and quality of life of patients with chronic obstructive pulmonary disease, and the intervention methods of pulmonary rehabilitation. The future trend was predicted as the clinical application value of pulmonary rehabilitation in the treatment of lung cancer.Conclusions:At present, pulmonary rehabilitation research is in a stable development period. However, the cooperation between domestic authors and institutions is not close enough compared with foreign countries. In the future, China should strengthen the communication and cooperation between research teams, and learn from foreign research results to further develop the application of pulmonary rehabilitation in lung cancer patients, to promote the development of pulmonary rehabilitation research in China.

The purpose of this paper is to review the research status on non-suicidal self-injury (NSSI) in children and adolescents, so as to provide references for the cognition and intervention of NSSI behavior in children and adolescents. NSSI behavior is an independent risk factor for suicide, which not only poses a serious threat to the mental health of children and adolescents, but also brings a great burden on families and society. This paper focuses on the epidemiology, etiology, relationship with other psychiatric disorders, and treatment of NSSI behaviors in children and adolescents.

ABSTRACT

Angong Niuhuang Pills, a classical formula in traditional Chinese medicine, are lauded as one of the "three treasures of febrile diseases" and have been widely used in the treatment of diverse disorders with definite efficacy. However, there is still a lack of bibliometric analysis of research progress and development trend regarding Angong Niuhuang Pills. Research articles on Angong Niuhuang Pills in China and abroad(2000-2022) were retrieved from CNKI and Web of Science. CiteSpace 6.1 was used to visualize the key contents of the research articles. In addition, the research status of Angong Niuhuang Pills was analyzed by information extraction to allow insight into the research trends and hotspots about Angong Niuhuang Pills. A total of 460 Chinese articles and 41 English articles were included. Beijing University of Chinese Medicine and Sun Yat-Sen University were the research institutions that have published the largest amount of research articles in Chinese and English. The keyword analysis showed that the Chinese articles focused on cerebral hemorrhage, stroke, neurological function, coma, cerebral infarction, craniocerebral injury, and clinical application, while the English articles focused on the mechanisms of cerebral ischemia, stroke, heavy metal, blood-brain barrier, and oxidative stress. Stroke, blood-brain barrier, and oxidative stress were presumably the research hotspots in the future. At present, the research on Angong Niuhuang Pills is still in the developing stage. It is necessary to highlight the in-depth research on the active components and mechanism of action and carry out large-scale randomized controlled clinical trials to provide references for the further development and application of Angong Niuhuang Pills.

ABSTRACT

Brain-computer interfaces (BCIs) have become one of the cutting-edge technologies in the world, and have been mainly applicated in medicine. In this article, we sorted out the development history and important scenarios of BCIs in medical application, analyzed the research progress, technology development, clinical transformation and product market through qualitative and quantitative analysis, and looked forward to the future trends. The results showed that the research hotspots included the processing and interpretation of electroencephalogram (EEG) signals, the development and application of machine learning algorithms, and the detection and treatment of neurological diseases. The technological key points included hardware development such as new electrodes, software development such as algorithms for EEG signal processing, and various medical applications such as rehabilitation and training in stroke patients. Currently, several invasive and non-invasive BCIs are in research. The R&D level of BCIs in China and the United State is leading the world, and have approved a number of non-invasive BCIs. In the future, BCIs will be applied to a wider range of medical fields. Related products will develop shift from a single mode to a combined mode. EEG signal acquisition devices will be miniaturized and wireless. The information flow and interaction between brain and machine will give birth to brain-machine fusion intelligence. Last but not least, the safety and ethical issues of BCIs will be taken seriously, and the relevant regulations and standards will be further improved.

ABSTRACT

With scarce resources, natural Bovis Calculus is expensive and hard to meet clinical demand. At the moment, four kinds of Bovis Calculus are available on the market: the natural product, in vitro cultured product, synthesized product, and the product formed in cow after manual intervention. In this study, papers on the four kinds of Bovis Calculus products and relevant Chinese patent medicines were searched from Web of Science, PubMed, and China National Knowledge Infrastructure(CNKI). CiteSpace, citexs AI, and CNKI were employed for bibliometric analysis and knowledge map analysis. On this basis, the status, trend, and focuses of research on Bovis Calculus and relevant Chinese patent medicines were summarized. The results suggested overall slow development in the research on Bovis Calculus and relevant Chinese patent medicines with three typical growth stages. It is consistent with the development of Bovis Calculus substitutes and the national policy for the development of traditional Chinese medicine. At the moment, the research on Bovis Calculus and relevant Chinese patent medicines has been on the rise. In recent years, there has been an explosion of research on them, particularly the quality control of Bovis Calculus and the Chinese patent medicines, the pharmacological efficacy of Chinese patent medicines, such as Angong Niuhuang Pills, and the comparison of the quality of various Bovis Calculus products. However, there is a paucity of research on the pharmacological efficacy and the mechanism of Bovis Calculus. This medicinal and the relevant Chinese patent medicines have been studied from diverse perspectives and China becomes outstanding in this research field. However, it is still necessary to reveal the chemical composition, pharmacological efficacy, and mechanism through multi-dimensional deep research.

ABSTRACT

Objective:To understand the current research status of master of professional degree students in clinical medicine in provincial teaching hospitals with moderate scientific research level under the " dual-track" training system.Methods:Sojump online survey was conducted to investigate the cognition, current situation and self-evaluation of all clinical master degree students in three grades in a provincial teaching hospital, and the research status of professional degree and scientific degree students was compared and analyzed respectively.Results:The proportion of scientific degree students participating in scientific research projects was significantly higher than that of professional degree students. The proportion of professional degree students participating in scientific research projects was still not high even in the third year of graduate students. However, there was no difference between scientific degree and professional degree students in the publication of scientific research papers. The scientific degree of scientific research knowledge is significantly higher than that of professional degree students. Although scientific degree students receive more scientific research guidance from their supervisors, professional degree students communicate more with their supervisors, and the results show that professional degree students are significantly more satisfied with their supervisors, graduate policies and scientific research policies than scientific degree students. In addition, the results of graduate students found that the degree of research stress of both scientific and professional degrees exceeded 50%.Conclusions:Scientific degree is better than professional degree in research status because of professional characteristics and more research guidance from their supervisors. However, through the reform of professional degree training mode based on scientific research project management in recent years, professional degree students have been able to communicate more with their supervisors, and their satisfaction with their supervisors and colleges has significantly increased. In addition, sufficient attention should be paid to high scientific research pressure of medical graduate students.

ABSTRACT

Cardiac arrest is the fourth stage of sudden cardiac death, which is characterized by the cessation of electrical activity in the heart, rapid circulatory and respiratory failure, and the prognosis is often poor. How to effectively predict cardiac arrest is the key and difficult point in the diagnosis and treatment process. In recent years, the research on the application of early warning scoring system in cardiac arrest has made continuous breakthroughs, from initially formulating a traditional scoring system containing only basic vital signs indicators according to a certain number of samples to continuously increasing and changing indicators, increasing the sample size, and formulating an improved scoring system with better sensitivity and specificity. Nowadays, with the continuous development of electronic information technology, machine learning technology is introduced into the formulation of scoring system, which breaks through the limitations of previous scoring system and has achieved good results in clinic. This article analyzes and compares the relevant research and cutting-edge progress of different early warning scoring systems at home and abroad, and summarizes the research results, gaps and shortcomings. Finally, combined with the relevant policies of graded diagnosis and treatment in China, this paper discusses the development and application direction of cardiac arrest early warning scoring system in the future.

In recent years, with the continuous studies on tumor metabonomics, more and more results have shown that changes of metabolism play important roles in the occurrence and development of malignant tumor. Carcinogenic factors can destroy the metabolic balance of human body, induce metabolic reprogramming, and then mediate a variety of biological behaviors to partici-pate in the proliferation and invasion of cancer cells. Lipids provide the body with the necessary energy and essential fatty acids, and a variety of lipid molecules and metabolites are involved in cell signal transduction. Lipid metabolism is an important link in the metabolic system of the body, and the relationship between the occurrence and development of pancreatic cancer and lipid metabo-lism is not clear. The purpose of this paper is to reveal the changes of lipid metabolism in pancreatic cancer, summarize some preclinical studies and clinical trials, and deeply explain the research status of abnormal lipid metabolism associated with pancreatic cancer, so as to provide new ideas for the study of pancreatic cancer pathogenesis and accurate treatment.

ABSTRACT

At present, many scalp acupuncture schools for cerebral palsy include Jiao's scalp acupuncture, Jin's scalp acupuncture, international standard scalp acupuncture and Zhu's scalp acupuncture. Each school of scalp acupuncture has its own strengths. And there are subtle differences in the theoretical bases and acupoint selection standards, but they also have similarities. Some acupoints selected by various schools in the treatment of children with cerebral palsy are mostly adjacent or identical in the corresponding position of the cerebral cortex, and the curative effect is obvious in improving the spasm degree, motor function and language of children with cerebral palsy. Single scalp acupuncture or scalp acupuncture combined with other therapies can achieve better results. In the future, it is necessary to standardize the clinical treatment plan, compare the efficacy of different schools, improve the follow-up and tracking, and deepen the basic research, so as to promote the application and promotion of scalp acupuncture in clinical treatment of cerebral palsy.

ObjectiveTo understand the current status and development of research on the subject of domestic violence, and to grasp the main research hotspots in the field. MethodsThe literature in the core database of Web of Science were retrieved, and 20 417 literature related to domestic violence from January 1966 to September 2021 were sorted by using the CiteSpace software, and the number of literature, authors, research institutions and keywords were analyzed. Results①The volume of literature on domestic violence grown rapidly since 2015. ②Among the authors, Kelsey Hegarty, Gene Feder and others had large number of publications on domestic violence, moreover, the authors' cooperation formed some small collaborative groups and research groups of a certain scale. ③The research institutions were closely collaborating, among which University of North Carolina was the institution of highest number of publications, and Boston University was the institution with the strongest research centrality. ④Hot keywords kept developing and changing, and the current hot keywords were mainly dating violence, sexual violence and the perception and perpetration of domestic violence. ConclusionThe field of domestic violence has been studied by a large number of researchers and has given rise to many research hotspots.

ABSTRACT

Objective:To analyze the characteristics and hot spots of hospital management projects funded by National Natural Science Foundation of China(NSFC), for the reference for furthering improving the modernization level of hospital management.Methods:The NSFC funded research projects in hospital management during 2010-2020 was searched. Project name, keywords, approval year, affiliation and other informations were obtained. CiteSpace was used for bibliometric analysis of these research projects, while all the data acquired were subject to descriptive analysis and frequency analysis.Results:The number of hospital management projects funded by NSFC increased from 4 in 2010(0.93 million yuan) to 32(9.94 million yuan) in 2020, totaling 192 projects and 72.76 million yuan. These numbers consisted of 90 general projects(46.9%) and 79(41.1%) youth fund projects respectively. All the funded projects were distributed in sub-disciplines under the NSFC first-level discipline of macro management and policy, namely health management and policy(154) and medical/drug management and policy(38). The hotspots of the funded projects covered performance evaluation, payment method, public health system, health economics, hierarchical diagnosis and treatment, utility theory, regional medical alliance, essential medicine system, capitation and incentive strategy, and so on.Conclusions:Recent years are witnessing a rising trend in the number and grants of NSFC projects on hospital management. More and more funds are granted to universities, especially young researchers, and a hospital management research system has been initially taken shape. Hospital management in China should take into full account of the management characteristics and the status quo of the medical and health field, focus on performance appraisal incentive mechanism, hierarchical medical system, construction of medical alliances. and patient medical experience as well. In addition, the cooperation between hospitals and universities should be strengthened in order to promote the translation of research results.

ABSTRACT

Breast cancer, as a kind of malignant tumor with high incidence rate in female population, poses a great threat to people's health. At present, chemotherapy is the main treatment for breast cancer besides surgery and radiotherapy. However, chemotherapy is often accompanied by multidrug resistance (MDR), which results in a series of severe consequences such as low efficacy, relapse of cancer after drug withdrawal, increased consumption of medical resources, and increased burden of medical system. Previous studies showed that the mechanism of MDR in breast cancer was quite complex, involving drug efflux, deoxyribonucleic acid (DNA) damage repair, tumor microenvironment, autophagy, epigenetic regulation, tumor stem cells, lipid metabolism, and so on. In addition, there were extensive connections among the mechanisms of MDR. Therefore, it is necessary to reveal the mechanism of MDR and develop corresponding drugs to reverse MDR, thus improving the effect of chemotherapy on treating breast cancer. Chinese medicine has the advantages of high efficiency, low toxicity, multi targets, and overall regulation. Various studies found that Chinese medicine monomer, single drug, and compound were able to reverse MDR in breast cancer by regulating the expression of drug efflux protein, promoting apoptosis, and regulating autophagy, showing the potential of anti-MDR in breast cancer. This paper summarized the research progress of the mechanism of MDR in breast cancer and the strategies of Chinese medicine in coping with MDR in breas cancer in recent years, and provided references for further research.

ABSTRACT

The pharmacokinetics of traditional Chinese medicine is a subject that studies the dynamic changes of the absorption, distribution, metabolism and excretion of complex components of traditional Chinese medicine, which is an important method for elucidating the pharmacodynamic material basis, action characteristics, and compatibility mechanisms of traditional Chinese medicine. However, given on the fact that traditional Chinese medicine is a multi-dimensional and complex system with multiple components, multiple pathways, multiple targets, and an unclear pharmacodynamic material basis, the research on the pharmacokinetics of traditio-nal Chinese medicine has become a scientific and technological problem. Although the pharmacokinetics of traditional Chinese medicine has achieved remarkable development with the emergence of new theories, methods and technologies, there are still some problems in the application of the research direction of the pharmacokinetics of traditional Chinese medicine judging from the current application of the National Natural Science Foundation of China. Therefore, this article discussed the current research status on pharmacokinetics of traditional Chinese medicines by analyzing the projects funded by the National Natural Science Foundation of China in the past 5 years from 2016 to 2020, mainly including the application and funding analysis, main research contents of the projects in pharmacokinetics of traditional Chinese medicines. And the research hotspots, difficulties and deficiencies were focused in order to provide certain reference for researchers engaged in pharmacokinetics of traditional Chinese medicine.

ABSTRACT

Xihuangwan is composed of four Chinese medicines: Bovis Calculus, Olibanum, Myrrha, and Moschus. Modern pharmacology studies have shown that Xihuangwan has anti-inflammatory, antibacterial, anti-tumor, anti-mammary gland hyperplasia effect, and can enhance the body's immune function. Cancer seriously endangers public health and safety-of-life, and is a major cause of mortality of Chinese citizens. It is a disease with intricate etiopathogenesis caused by the joint action of circumstances and hereditary factors. At present, anti-tumor chemotherapy drugs in clinical application not only have toxic and side effect, but also affect clinical efficacy and prognosis of patients. Long-term use will also lead to drug resistance of tumors. As a traditional classic anti-cancer prescription, Xihuangwan has been used more and more in tumor research with the rise of Chinese medicine culture. It is provided with remarkable inhibitory effect on liver cancer, gastric cancer, carcinoma of the lungs, mammary gland, colorectal carcinoma and other malignant tumors. In clinical practice, Xihuangwan , mostly used as adjuvant drugs in combined use with chemotherapy drugs for anti-tumor effect, can reduce the side effect of chemotherapy drugs and the untoward reaction of sufferers, improve the survivability of patients to chemotherapy, reduce or delay postoperative tumor recurrence, enhance the body's immune function, and reverse the tolerance of tumor cells. Based on the anti-tumor research of Xihuangwan, we summarized its mechanisms in inducing cell apoptosis, regulating amino acid metabolism, reversing drug resistance, interfering with cell cycle, resisting tumor metastasis and invasion, regulating immune function, improving tumor microenvironment, and regulating signal pathways, as well as its clinical combination with chemotherapeutic anti-tumor drugs, analyzed the current anti-tumor research status of Xihuangwan's research, and put forward the shortcomings and unresolved problems in order to provide theoretical basis for further research and clinical application of Xihuangwan.

Objective:To review and summarize the current research status of traditional Chinese medicine(TCM) for the treatment of chronic atrophic gastritis(CAG),provide references and hints for relevant studies,and contribute to the further understanding of TCM and the application of TCM in the treatment of CAG with scientific evidence. Method:The PubMed and Web of Science databases were searched for relevant literature on the treatment of CAG with TCM from their establishment to August 31,2020. Eligible randomized controlled trials (RCTs) and animal studies were included according to the inclusion and exclusion criteria,and then the information of the included studies was extracted,summarized,and organized for further analysis. Result:A total of 4 RCTs and 21 animal studies (including 13 papers on compound studies,3 papers on single herb studies,and 5 papers on monomer studies) about TCM treatment for CAG were included in this study. RCTs showed that TCM could work well in improving the pathological state of gastric mucosa and clinical symptoms in patients. However,there were problems of low study quality,and non-uniform diagnostic criteria for gastric mucosal pathology and clinical efficiency evaluation. Animal experiments mainly focused on the study of drug mechanism exploration,and their results showed that TCM treatment of CAG was characterized by multi-target action. However,the animal experiments also had some problems such as inconsistence of CAG animal model establishment,positive drug selection,drug intervention methods as well as intervention cycles among different experiments. Conclusion:The efficacy of TCM in the treatment of CAG has gradually gained global recognition,but there is still a need for further standardization and unification of research methods. In the future,high-quality clinical trials and standardized animal experiments are still needed to conduct in-depth studies on the time for intervention,intervention methods,active ingredients and mechanisms of TCM,so as to make contributions to the full understanding and application of TCM in the treatment of CAG.

ABSTRACT

The homology of medicine and food has long been recognized in China, and the medicinal and edible resources are often employed to prevent and treat diseases or maintain health in traditional Chinese medicine (TCM). Due to the unique geographical and climatic conditions, the Xinjiang Uygur Autonomous Region (hereinafter referred to as Xinjiang) is blessed with abundant medicinal and edible resources, like Rosae Rugosae Flos, Punica granatum, and Amygdalus communis, which have been widely used by local ethnic communities as medicine in light of the remarkable pharmacological activities, guaranteeing their health condition to some extent. This paper collected the relevant articles from China National Knowledge Infrastructure (CNKI) database through the keyword and full-text search with the medicinal and edible resource Rosae Rugosae Flos as the search term, and the subsequent analysis revealed that the medicinal and edible resources in Xinjiang has a wide range of applications in food, medicine, and ecological protection. This paper further explored their research value, status, and prospects, so as to provide some references for the rational, effective, and sustainable development and utilization of these medicinal and edible resources in Xinjiang, thus better achieving "poverty alleviation" and "rural revitalization", popularizing TCM culture, and building a healthy China.
